Submit your proxy
The New York State Insurance Fund (NYSIF) updated new rules and regulations for safety group members. As a result, members must complete a new proxy. You may remember completing a proxy annually – this new proxy only needs to be completed once. Please fill out the proxy and return it to us at your earliest convenience.
Ways to complete the proxy:
Digitally: Complete the form online, print, sign and return via email, fax, or mail.
Manually: Print, fill out, sign, and return via email, fax, or mail.
Please note: Original signatures are required. NYSIF will not accept typed signatures. If you would like to complete the form and sign with an Adobe signature, simply email us a request and we will send you an electronic version to e-sign.
